The following topics are directly (or indirectly) related to spirituality. Which topics do you find important, and on what do you wish to focus?

At the bottom of this questionnaire you can list areas I didn’t mention as this is about your spirituality, which may not align with these topics. Don’t struggle too much with this, it is designed to allow us to explore what matters to you.

After rating each topic’s importance, look over the list again and choose five or so topics you’d most like to focus on in our sessions. Circle that topic’s name and rank your five or so choices in order of importance (1 for the topic you “most wish to explore,” 2 for the topic you “second most wish to explore,” and so on). Again this is only to point us in a direction; we won’t necessarily need to commit to these.
